Job Description

Primary Duties & Responsibilities include, but are not limited to:

  • Ensure the triage and assessment of all clients upon admission to the Harbor House programs, in accordance with established criteria.
  • Communicate with Medical Director and obtain MD’s approval for clients’ medical assessment and required patient care by performing examinations and tests, administering medication, treating wounds and a myriad of other duties.
  • Act as a liaison between patients and doctors and answer questions or offer guidance for patient families. 
  • Complete Initial medical Assessment within 3 days of admission, and consult MD or Psychiatric team as medically indicated. 
  • Ensure the completion of necessary admission forms which includes list of medications and medical assessments. 
  • Initiate and follow-up observations:
  • Ensure follow-up observation is conducted daily for rehabilitation clients,. 
  • Recruit, train, monitor and evaluate nursing staff and auxiliary medical staff
  • Schedule nursing and auxiliary medical staff to ensure daily coverage,
  • Assign tasks to nursing and auxiliary staff and ensure completion of tasks assigned,
  • Review MARS and train staff if MARS are not recorded in accordance with OASAS and DOH regulations,
  • Initiate communication to resolve problems with pharmacies, medical providers and entitelement specialists.
  • Train all staffers on client medication self-administering protocol.
  • Continue narrative in the progress notes of client’s chart regarding status, condition and participation.
  • Request and implement the transfer of medically or psychologically distressed clients to appropriate services, utilizing the established procedures.
  • Maintain a safe, therapeutic, physical and social environment for the client undergoing SUD and psychiatric symptom stabilization and transitional care.
  • Participate in weekly case conferences.
  • Ensure the delivery of weekly health education seminars.
  • For all new admission, conduct complete physical assessment within 3 days of admission. Review any medical feedback from primary care providers and specialists.
  • Arrange for MD/NP to prescribe refills of routine medication. 
  • Monitor medication necessary for clients’ MAT and/or refer them to off-site MAT services.
  • Ensure that controlled substances are stored and that distribution is recorded as per program regulations.
  • Initiate communications with Oversight Supervisor and Medical Director when there are medical staffing issues/concerns. 

Job Requirements


Qualifications/Criteria:
  • Valid NYS Registered Nursing License or Nurse Practitioner.
  • Knowledge of generally accepted nursing practices and principles as relevant to addiction services.
  • Two years of experience in behavioral health setting.
  • Must possess and demonstrate strong teaching and technical competence.
  • Three+ years nursing experience.
  • Bachelor’s Degree in Nursing required.
  • Knowledge of EMR’s
  • Willingness to work a flexible schedule and fill in when needed.
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Proficiency in maintaining medical records and preparing clear and accurate patient care plans consistent with OASAS 820 guidelines.
  • Good written and oral communication skills.
