Job Description
Title: Social Services Coordinator
Location: Boca Raton, FL
Schedule: 8:30 – 5:30 M-F
Compensation: $30hr
Role Overview
The Social Services Coordinator is responsible for building strong, trusting relationships with residents and families while identifying needs, resolving concerns, and connecting residents with appropriate resources.
Working closely with the interdisciplinary team, this position plays a key role in supporting residents as they transition throughout the continuum of care, including Independent Living, Assisted Living, Memory Care, Respite, Skilled Nursing, and Rehabilitation.
The Social Services Coordinator assesses residents’ social, wellness, and healthcare needs and coordinates access to internal and external resources to ensure appropriate, seamless transitions and resident-centered outcomes.
This role also supports wellness and healthcare programming and serves as a key liaison between residents, families, caregivers, community departments, and outside resources.
Your Contributions
- Support cross-functional coordination of post-acute services throughout the community.
- Monitor and help ensure residents are receiving the appropriate level of care based on their changing needs.
- Collaborate with interdisciplinary team members to identify and recommend additional services or transitions within the continuum of care.
- Monitor hospitalizations, ER visits, resident falls, and significant incidents in collaboration with the EMR department.
- Follow up with residents following hospital discharge and provide appropriate resources and support.
- Address resident and family concerns and direct issues to the appropriate department.
- Participate in interdisciplinary meetings, care coordination meetings, Assisted Living stand-up meetings, wellness committees, support groups, and other relevant community meetings.
- Create and maintain daily reports regarding resident cases, needs, and transitions.
- Maintain and regularly update resident information within the shared Transitions List for interdisciplinary team access.
- Promote internal services and programs designed to support resident wellness, engagement, and retention.
- Participate in new resident orientations and introduce residents and families to the services and resources available through Social Services.
- Encourage resident participation in community programs, activities, and social events.
- Continuously evaluate residents’ physical, emotional, social, and wellness needs through observation and referrals.
- Serve as a resident liaison by coordinating assessments, consultations, and internal and external resources.
- Communicate with residents and families when changes in condition require additional services or a transition to another level of care.
- Advocate for residents and facilitate communication between residents, families, caregivers, and staff.
- Coordinate services with internal departments and external providers.
- Assist residents with understanding health plan benefits, general medical insurance questions, Advanced Directives, and related documentation.
- Maintain accurate and confidential resident records and documentation.
- Develop and maintain a current network of internal and external health-related resources and service providers.
- Coordinate with Emergency Medical Responders and communicate applicable policies and resources to residents and families.
- Plan and coordinate health education programs and support groups.
- Network with qualified educators, instructors, and healthcare professionals to provide wellness programming in coordination with the Activities Department.
- Remain actively engaged in broader community needs and support the organization's philosophy and programs.

- Maintain required licensure, certifications, and health screenings.

- Participate in professional development and continuing education.
- Perform other duties as assigned by the Supervisor.
Bringing to the Table
- Bachelor’s degree or higher in Social Work or a related field.
- Previous Assisted Living experience is required.
- Experience working with senior living residents, families, caregivers, and interdisciplinary teams preferred.
- Strong interpersonal, communication, organizational, and problem-solving skills.
- Ability to demonstrate empathy, professionalism, and sound judgment when working with residents and families.
- Ability to coordinate multiple priorities and work effectively across departments.
Physical Requirements
- Ability to sit and walk intermittently throughout the workday.
- Frequently required to walk, stand, bend, and move throughout the community.
- Must be able to communicate effectively and hear well.
- Good vision is required, including close, distance, peripheral, and depth perception, as well as the ability to adjust focus.
- Ability to lift and/or move items weighing more than 25 lbs. with appropriate assistance and equipment.
- Ability to bend, stand, walk, and assist across various community units.
- May occasionally assist residents with transitioning from a seated to standing position.
